More Information for Identity Verification

We need to verify your identity for disability benefits. Your information will be kept confidential. Citizenship and immigration status do not affect eligibility.

If you mail documents after the due date, include an explanation so we can determine if you had good cause for the delay.

Gather Your Documents
For us to verify your identity, you must provide two documents:
  • One photo identification document that shows your full name and date of birth.
  • One additional identity document. Refer to the list below.

Below are more examples of documents you can use to verify your identity.

Photo Identification
Your photo identification must not be expired and show your photograph, full name, and date of birth.
Acceptable documents include a copy of your:
  • Identification card (state or municipal)
  • Driver license (state, including REAL ID and AB 60 driver license, or foreign)
  • Passport (US or foreign)
  • Permanent Resident Card (Green Card)
  • Certificate of Naturalization
  • US nonimmigrant visa
  • Border Crossing Card (DSP-150)
  • Citizenship card (US or foreign)
  • Consular card
  • Employment Authorization Document
Additional Identity Document
Another identity document, such as:
  • Proof of your residence as shown on the enclosed DE 4365 DI-T.
  • A copy of your Social Security Statement. The Social Security Administration (SSA) mails this each year, or you can download from ssa.gov Do not use your Social Security card.
  • Your tax form or pay stub for the year before your claim began. It must include your full name, Social Security number, name of the employer, and the pay period or date it was issued.

Important: If you received this notice, but did not apply for disability benefits, you should report this as fraud. Report fraud at askedd.edd.ca.gov or call the EDD Fraud Hotline at 1-800-229-6297.
