We are currently reviewing your Disability Insurance (DI) claim for medical provider verification. Disability claims are not completed until all requirements are satisfied, including establishing medical eligibility through a physician/practitioner certificate signed by a duly licensed provider.1
This impacts your disability claim:
By law, your claim may be suspended for up to 90 days as we verify your medical provider. If verification is not received from your medical provider, your disability claim will be disqualified. If this occurs, you will receive a Notice of Determination you can appeal. Otherwise, benefits will be paid upon successful verification.
We will contact your medical provider with instructions on how to complete the verification process.
You may contact your provider and alert them of communications from us regarding your disability claim.
If you received this notice but did not file a claim, you may report benefits fraud at edd.ca.gov/about_edd/fraud. You can also call the EDD Fraud Hotline at 1-800-229-6297.
